Image/Video /Audio Use Policy: Mill Valley Little League (MVLL) requires an image/video/audio release form from the subject of any image /video/audio used on its web site and social media platforms, including Instagram, Gamecast, and Facebook. MVLL will not publish any image/video/audio of a minor unless his or her parent or legal guardian has clicked on the checkbox at the bottom of this waiver and release form.

Waiver and Release Form: By checking the checkbox below, I hereby give permission to MVLL to use photographs, voice recordings, or video taken of me during the games and events associated with MVLL in any manner to help promote the league activities as determined in the sole discretion of MVLL. Such use could include publications, media releases and announcements, electronic or otherwise, and on MVLL websites or social media pages. I understand that neither I nor my child/ward will receive any compensation if such image appears in any of the manners listed above or any other manner that the league deems appropriate. I agree that such image is the property of MVLL. I understand that no names will be used on the web site unless specific permission, in written form, is given. I understand that once any image/video/audio is posted to the website and social media platforms the image/video/audio can be downloaded by any computer user around the world. I hereby attest that I am the legal parent/guardian of the child(ren) listed below. This consent is effective until such time as I revoke it in writing and provide a copy of the revocation to MVLL.
